If you demand for the game to be removed from the service, your application must have the following information:
1) name and contact information;
2) the specific name of the content that is to be removed as well as it's specific location (=game name and URL);
3) your assurance that the content in question is by your best knowledge indeed illegally available in the service;
4) assurance that your efforts in asking for the content provider/content submitter to remove the content have been without result or that you haven't been able to contact the content provider/content submitter;
5) your assurance that you are the owner of rights for the content or that you represent the owner of the rights to the content.
